Did you know you have the right to see your medical records in the State of California. Doctors,HMO's and hospitals have to let you inspect your medical records during Business Hours within (5) working Days after Receiving a written request from you. The paitent is requiired to pay a reasonable fee for the search of such records. You also have the right to write a statemnet of up to 250 words in regards to any information that is in your records that you think is incorrect this statement will be part of your record and file.
You can request copies of your records your health care ,doctors and hospitals have to provide you with copies of the records , they the providers have the right to charge you up to 25 cents per page, or 50 cents if the records comes from moicrofilm. Any company that keeps medical records for the purpose to make them available to the public or health care providers for diagnosing and treating have to provide you a copy of your records at no charge.
All most all of the time a consent is required before your medical records my be released, this under the California Confidentiality of Medical Information Act
A true authorization for release of medical records , should include a handwritten or typewritten no smaller than 8 point type request ,seperate from any other parts of a forms, and have a line for you to sign. The form should be spefic about use and limitations of information to be relaesed, It should specify the name and functions of the doctors or health oraginazation that you are oking to release your records. There should be wording for the specific us of your information and a date that the authorization is declared null and void
