It is my observation, looking for replacement heads for my Braun electric toothbrush, that the 4-packs are the counterfeit ones and the 3-packs are the original Braun replacements.  I got my initial info from the two other informative guides and have figured this out based on my experience with real replacement packs.  Good luck!
